PROGRAM OVERVIEW
The CCSD Transition Academy serves young adults, ages 18-22 in a functional transition program. This program is developed with the purpose of providing young adults with cognitive disabilities the opportunity to receive realistic vocational and functional life skills instruction to develop skills needed to successfully transition into a more independent life. There are three distinct programs at the Transition Academy to support students with varying transition outcome goals

PURPOSE OF THE PROGRAM
The purpose is to provide young adults with cognitive disabilities the opportunity to receive realistic vocational  experiences, while developing skills needed to successfully transition into a more  independent life.

GRADUATION
Transition Academy students participate in the Graduation Ceremony and related senior activities in the spring before they come to the Transition Academy. Diplomas will be awarded after the completion of the program, or upon the age of 22.

ENTRANCE
High school seniors who have completed four years of high school, participated in GAA, have completed academic diploma requirements, have had access to campus-based domestic, daily living, and recreational activities or access to weekly community based work experiences are eligible to participate in the Transition Academy Enrollment Process.

SCHOOL STAFF

School personnel working in the Transition  Academy will receive training to implement components of the program in the community as well as in the school.  School personnel will provide supervision and training in the school environment as well as in the community.

COMMUNITY JOB SITES

Our students will be working at job sites in the community based on their individual strengths,  preferences, interests, and site availability.
